On the afternoon of Saturday, October 4, 2025, a multi-vehicle crash occurred on State Route 28 in the Town of Shandaken, prompting a response from New York State Police stationed in Kingston and Wurtsboro.
Initial findings indicate that a 2018 Freightliner, traveling eastbound and driven by 41-year-old Oscar A. Madrid of Somerset, New Jersey, struck a 2020 Mazda. The impact forced the Mazda into the westbound lane, where it collided with a 2025 Subaru.
The Subaru’s driver, a 37-year-old man, and his 36-year-old female passenger—both from Wappingers Falls, New York—sustained minor injuries. They were transported to Kingston Health Alliance Hospital for medical evaluation.
More serious injuries were reported in the Mazda. The 28-year-old male driver and his 35-year-old female passenger, both from Newburgh, New York, were airlifted to Albany Medical Center. The female passenger is currently in critical but stable condition, while the driver is in stable condition.
Neither the Freightliner’s driver, Oscar A. Madrid, nor his 33-year-old male passenger from Fords, New Jersey, reported any injuries.
Following the incident, Madrid was taken into custody and charged with second-degree reckless endangerment, reckless driving, and multiple traffic violations. He was processed at the State Police barracks in Kingston and later released with an appearance ticket. A court date has been scheduled for October 28, 2025, at 7:00 p.m. in the Town of Shandaken Court.
Authorities are continuing to investigate the collision.
